Oh, I meant in the previous comment to just not have the world map and have a solid square with a pin on it in its place. Hope that made it clear.

We were looking for something more like just a large white square with a pin shaped negative space in the middle. Would still like to keep the checkboxes, though

Is it possible to replace the checkboxes on the bottom with a miniature map and perhaps a location pin of some sort?

Is it possible to change the abstract graphic to just one line on top with 3 or 4 square dots under

I appreciate the boldness of the colour gradient. I think looks great on its though I think it might be difficult for us to apply this in the right way in a lot of our materials

The "question sheet" conveys the ideas of surveys well, but the lines of "text" look a little more like a paragraph than a survey question. The check mark on the "A" also seems a little out of place in its current form.
