What We Do
Pathmaker Health is a provider based healthcare organization serving women and children in the Medicaid population. The audience is both patients and health care clinics/physicians.
Leaning into the whole health model, we create a path for women and children to live healthier and supported lives. We help them find their path through a combination of clinical services and community resources anchored on FQHC’s as the core of our service model and enhanced by partnerships with select provider organizations and health systems

Very open to creative ideas... the only idea we have considered so far is an image of a woman & child with steps making a path. Not sure if that would work, and open to others.
