Personalized Diabetes Care 24/7

We are a first-in-class diabetes care clinic providing 24/7 care to our patients on a membership basis. We provide on-site office visits as well as telemedicine video, email, text, and phone-based care. Our services combine traditional medical care with a focus on wellness and lifestyle education. Our prospective members are individuals with Type 2 diabetes (very common, 90% of diabetics), Type 1 diabetes (less common, 10% of diabetics), and women with gestational diabetes (diabetes that develops during pregnancy). We also serve people with prediabetes, a condition that leads to diabetes for 70% of people withing 5 years. The ages of our patients range from 16 to 86, but our primary membership consists of individuals age 30-65, with household incomes of $60,000 to $200,000. Many members have limited insurance or a high-deductible insurance plan with large out-of-pocket co-pays. For most of our patients, diabetes is their main medical problem.

We are very open to any idea or concept - it needs to work well across website, social media, business cards, and stationary and faxes. Our logo icon will also show up on our medical uniforms such as medical scrubs and white coats. It will show prominent in signage on the exterior of our building. Because we are not a typical healthcare clinic, but rather a pay-to-play membership, we invite and air of sophistication and exclusivity.
