Continuum Health CareLogo Design Contest

Our providers, physicians and nurse practitioners believe there is a current huge "gap" and continuity in healthcare is very "fractured" especially for the geriatric population. If these clients have to be in the hospital, they are typically assigned to different providers (not their primary care physicians), if they need to go to skilled/short term rehab, yet another provider is assigned to them. We are passionate about following our clients through the continuum of their care!
