This is my first choice. Please have the background person white(no color) like your original of this design. I'd also like one where the logo is on left, and the print off to the right

I'd make the patient form in pain grey in background. Maybe the rays in golden yellow? Brighter. Blue text and keep grey for the background person in pain? What do you think?
