I´ve made #514 with the colors your mention and other with another blue a bit more lighter #515 since the pantones 655 and 648 pantone color codes don´t differentiate to much.

Can you make the smile portion slightly thicker?

Can you put the blue smile below orthodontics somehow?

Can you make the "smile" (dark blue) portion on the circle slightly thicker? Also, can you try a different one with a capital M in the circle to match the M on McClain?
