I really like entry # 159. It is very simple and strong.
Can you please try to integrate some colour into the scheme? Maybe the word FORM is in a different colour.
Also can you make the letter PM a little bit easier to read. Maybe a slightly bigger gap between letters.

Yes no problem I will make new changes as you requested but you need to put me temporarily in top 5 design so I can upload.. in this part of the contest only the top 5 designers can upload changes... thanks!
