Like #18 but the tooth is too fat looking, try to make the tooth slender (skinner). Also read the update on the text layout.

remove the head (two circle on the tooth), make the tooth slender (skinner), and make the text on "Care Family Dental" little bigger, and my name littler smaller

I 've made the changes you asked.I 'll try also some san serif font.Maybe this will give a little bit more modern look.I 'll wait for your comments.
