What I'm trying to do is "box in" the patient

Now let's give the patient box the white background. Let's give the adhere box a gray background

I also think that Patient doesn't need the dots on either side. It should be made important by it's placement instead of dots. Can we also try it being in the center of the black hole?

I'm enjoying the gradient and the alternation of blue/gray. Could the patient somehow be more involved or pulled in by our gravity?

I like that patient is contain but the imbalance makes me read it as "Ad Patient Here Mangement". Could you fix that?

This is the most interesting of the new options but it visually off balance. Is there a way to make it less so?

A concept to play with: what if Patient was the word that was contained by the box? Since I'm keeping patients in the doctor's building

I like it and have 2 ideas. I would try them both separately and together
1. Can you add some sort of sizzle? Like a tweak on color or added piece to the design?
2. Can you try adding a dot or 2 to signify patients adhering to the practice because of gravity caused by the black hole?

how about this , any suggestion ?
